Office Hours

We are open from 7AM to 4PM Monday through Friday. Our early morning appointments are very popular and may need to be booked further in advance.

Insurance

We accept all insurance plans*. Please bring a copy of your insurance card with you to our office on each visit.

Appointments

New patients and some surgical patients are asked to arrive early, approximately fifteen to twenty minutes prior to your actual appointment time. This allows plenty of time for you to complete the necessary forms, including the Patient Information Form and Patient Health History Form. If you are not sure whether you need to arrive early for your appointment, please call our office staff. Please remember to allow for unpredictable traffic and give yourself a few minutes to spare. Your first appointment will last approximately an hour and a half, and will include an exam, consultation, and fee estimate. [back to top]

After Your Surgery

Payment

We offer a wide variety of payment options: major credit card, check, cash, or payment plans by Care Credit. [back to top]

Cancellations
